CCCHC Sliding Fee Scale

This program offers discounted pricing on office visits and services provided by our medical home team at CCCHC. Individuals with or without insurance may qualify. The program is based on household income and size and may reduce your costs anywhere from 20%‐100%. This program may benefit you and your family during the COVID‐19 pandemic. May is Skin Cancer Awareness Month

With more than 3.3 million people diagnosed annually, skin cancer is the most common form of cancer in the United States. Fortunately, skin cancer is also one of the most preventable forms of cancer and highly treatable when detected early.
