SMC Beulah Rehabilitation Department Relocation

Effective June 21, 2019 the SMC Beulah Rehabilitation Department will be housed at the CCCHC Beulah Clinic — 1312 Hwy 49 N, Beulah. Patients needing Rehabilitation Services should park and enter the facility from the south entrance. Please call 7018733271 to schedule appointments.

CCCHC Behavioral Health Department Relocation

Effective June 17, 2019 the CCCHC Behavioral Health Department will be located at the main Beulah Clinic — 1312 Hwy 49 N, Beulah. Please call 7018734445 to schedule all Behavioral Health appointments.

Emergency Medical Responder Course

Stepping On — Building Confidence and Reducing Falls

Stepping On is a 7‐week workshop that has been researched and proven to reduce falls in older people. The workshop will be held at the Energy Wellness Center in Beulah and will meet on Wednesday, April 3 to May 15; 10 a.m. ‐ 12 p.m. The workshop is FREE for adults 60 years and older. CCCHC Beulah Clinic — Construction Update

Temporary Relocation of Patient Entrances. The main entrance for patient visits at Coal Country Community Health Center — Beulah Clinic will be the southside vestibule. Please use this entrance for all services except Physical Therapy. Patient’s needing Physical Therapy, will park and enter the facility from the southeast entrance, enter the parking area from 12th St NW.
